prepared before arrival and your wait time will be minimized.
Items You Will Need for Online Pre-registration:
- All active insurance card
- Social Security numbers for patient and insurance subscriber
- Physician Order - information about your doctors, including names and
addresses
- Emergency contact information

- Any co-payment/deductible that can be made. (We accept
