At Saint Joseph Health System, we encourage all patients to pre-register. This way, your paperwork can be prepared before arrival and your wait time will be minimized.

Items You Will Need for Online Pre-registration:

  • All active insurance card
  • Social Security numbers for patient and insurance subscriber
  • Physician Order - information about your doctors, including names and addresses
  • Emergency contact information

Items You Will Need on Day of Admission:

  • All active insurance cards
  • Picture ID 
  • Any co-payment/deductible that can be made. (We accept MC, Visa and Discover
